Individualized Services
EPI uses a Personal Outcomes Planning process to individualize our services. We gain knowledge of an individual's dreams, preferences, ambitions, likes and dislikes, needs and desires through careful listening, observations and discussions.
The amount of service an individual receives may be scheduled for up to 24 hours a day, for a few hours each week or available in case of emergencies
EPI Community Living Adults services help people to learn some or all of the skills necessary to plan, shop for and prepare their own meals, do their own laundry, clean their own home and enjoy community events and activities.
EPI employees are available to assist with medical appointments, budgeting, and transportation. The services provided are flexible and defined individually through an interdisciplinary planning process.
Brain Injury Services
Exceptional Person, Inc.’s (EPI’s) Community Living Program offers an array of services designed to meet the unique needs of individuals living with acquired brain injury.
Using a Personal Outcomes Planning process, individuals identify ambitions, preferences, dreams, likes and dislikes, needs, and desires. Through individualized assessments, a plan is developed by careful listening, observations and discussions with the individual, professional(s) who know him/her and other persons important in their life. The plan provides a rehabilitative blueprint of how the individual wants to live today in order to achieve self-sufficiency into the future.
People receiving services are encouraged to become active in the community by healthy and productive lifestyle choices.
Home and Community Based Services range from a 24 hour service setting to an as needed hourly setting. Services are provided in neighborhoods close to public transportation so participants can easily access and participate in their community.
Strengths based programming is designed to provide each individual an opportunity to learn, re-learn and refine skills in areas such as:
- Healthy lifestyle development
- Social interactions
- Impulse control
- Behavior analysis & change
- Cognitive and orientation skills
- Self-management, self- motivation, self-care
- Home living skills
- Development of positive community connections
- Symptom & crisis management
- Goal setting & problem solving
