Wish List

We want to see each individual reach his/her full potential. We've come up with a wish list of items needed to help us do just that.

Your donations may be tax deductible. To make a donation from our wish list, simply fill out this gift-in-kind donation form and bring or mail it along with your donation to 760 Ansborough Avenue in Waterloo, IA 50701.

Donations benefiting multiple programs:

  • Tickets to local entertainment (hockey, baseball, basketball, festivals, theatre, museums, movies, etc.)

  • iPads - this tool is used to build skills, communication and entertainment.

Donations benefiting the adults attending the CHOICE Adult Day Habilitation program

Opportunities for physical movement and coordination:

  • Karaoke CDs (Party Time Karaoke, Country Classics, any familiar songs would be great)

  • New Play Station and games - another controller
